Person-Led Support Planning

Everything begins with person-centred and person-led planning. At HomePlace, we take the time to truly understand each person—their goals, interests, preferences, and support needs. The person we support leads the way, shaping their own vision for life.

Our role is to:

  • Help each person express their life goals
  • Coach and build their skills
  • Connect them with the right people, services, and community organisations to make their vision a reality

We focus only on the supports we do best from 1000’s of hours of experience:

  • Home and Living (including Supported Independent Living – SIL)
  • Community Participation
  • Innovative Shared Supports
