You will have a HomePlace Senior Coordinator.
A Senior Coordinator is a person who will help you plan what HomePlace support you need.
Your Senior Coordinator will work with you to make a schedule of support.
A schedule of support is a plan of your week.
Your schedule of support is based on when you want support.
Your Senior Coordinator will talk to you and your family or guardians.
Together, you will plan out what support you want to have.
This will happen once a year, or more often if you want it.
